Qualifications

Responsibilities

• Conduct individual/couples/family counseling with engagement and problem solving.
• Collaborate with community agencies for patient assessment and services.
• Evaluate and document intervention effectiveness and patient responses.
• Assess psychosocial, educational, and behavioral functioning; document updates.
• Respond to social work service requests; participate in treatment planning meetings.
