Job summary

Qualifications

Responsibilities

• Provides quality individual therapy focused on patient diagnosis and needs.
• Provides crisis intervention to new and current patients as needed.
• Conducts comprehensive assessments and formulates treatment plans.
• Provides case management and patient/family education.
• Makes referrals to hospital and external services to ensure discharge planning continuity.
• Participates in clinical supervision and peer review activities.
• Engages in community education and outreach to generate new business.
• Completes documentation within established timelines.
