• Strong communication and confidentiality, able to work independently or in a team.

Responsibilities

• Performs patient assessment using available information and patient input.
• Develops an individualized plan of care and updates it during visits.
• Identifies safety issues for patient and staff during visits.
• Reviews medication lists and provides education per provider directives.
• Coordinates with the healthcare team to ensure continuity of care.
• Participates in education and competency validation of clinical staff.
• Adheres to department standards of care and personalizes care as needed.
• Administers medications per provider orders.
• Demonstrates professionalism and completes required department competencies.
