Advocate Health in Wisconsin is seeking a licensed Physical Therapist to evaluate referrals and deliver comprehensive musculoskeletal, neuromuscular, cardiopulmonary, and integumentary assessments in an outpatient setting. You will establish treatment plans and monitor outcomes to meet patient goals.

Responsibilities include documentation, discharge planning, collaboration with physicians and teammates, and supervision of PT staff and assistants.

Formación

• Doctorate degree in Physical Therapy required.
• Licensed Physical Therapist in the state.
• BLS for Healthcare Providers within 90 days if required.

Responsabilidades

• Interpret physician referrals and perform musculoskeletal, neuromuscular, cardiopulmonary, and integumentary evaluations.
• Establish and implement individualized treatment plans to meet functional goals.
• Document care, progress notes, and required reports per policy; ensure productivity and billing compliance.
• Coordinate discharge planning, home programs, and patient education with patient/family and other care team members.
• Supervise PT assistants and rehab aides as applicable; participate in staff mentorship and continuing education.
• Engage in performance improvement activities to enhance patient outcomes and safety.
