Job summary

Advocate Health in Wisconsin seeks a full-time Outpatient and Inpatient Physical Therapist to evaluate patients, establish treatment plans, and drive optimal recovery outcomes. You will collaborate with physicians and teammates to ensure coordinated care and patient safety.

You will handle documentation, productivity standards, and ongoing competency, while supporting staff development and community health goals.

Qualifications

• Doctorate degree in Physical Therapy from an accredited school.
• Maintains continuing education requirements as mandated by state standards.
• Licensed Physical Therapist in the state of practice.
• Strong communication and organizational abilities.

Responsibilities

• Interpret physician referrals and conduct musculoskeletal, neuromuscular, cardiopulmonary, and integumentary evaluations.
• Establish and implement individualized treatment plans and adjust based on patient response.
• Plan for discharge with home programs, family teaching, and referrals as needed; coordinate care transitions.
• Complete documentation per policy and accreditation standards including evaluations and progress notes.
• Maintain productivity standards and ensure proper billing/documentation practices.
• Provide supervision or mentoring to assistants and rehab staff when applicable.
• Participate in performance improvement activities and accept coverage assignments as needed.
