Advocate Health in Wisconsin is seeking a licensed Physical Therapist for a full-time role in a small rural hospital setting. The position requires 40 hours per week with a schedule that includes Friday, Saturday and Sunday, and weekend premiums of an additional $11 per hour on those days.

Offer includes a $20,000 sign-on bonus for full-time new hires. Candidates must hold a Wisconsin Physical Therapist license and BLS certification within 6 months.

Qualifications

• Intermediate computer skills.
• Excellent communication and interpersonal skills.
• Demonstrated organization, decision-making, and problem solving skills.
• Certification Addendum and annual hospital-based certification requirements.

Responsabilités

• Interprets referrals and conducts musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations.
• Develops and implements a care plan based on evaluations and goals.
• Monitors outcomes and adjusts treatment plans; communicates with physicians.
• Plans discharge with home programs and patient/family instruction.
• Documents progress, billing, and scheduling per policy and ensures authorization per insurance guidelines.
• Assists with equipment setup and maintains a clean, safe treatment area.
• Uses modalities and equipment appropriately with knowledge of indications/contraindications.
• Participates in performance improvement activities and covers caseload needs.
• Engages in continuing education and mentors staff as requested.
• Provides age-appropriate care and adheres to department policies.
