Job summary

Carolinas Medical Center in Sheboygan, Wisconsin, is seeking a Physical Therapist for our Outpatient Rehabilitation clinic. The role covers evaluation, treatment planning, and discharge planning in a 40-hour work week, Monday–Friday, first shift. A sign-on bonus of $5,000 is offered along with benefits eligibility.

Licensure in Wisconsin and BLS must be obtained within 6 months if required by leadership. A Bachelor’s Degree in Physical Therapy is mandatory, with no prior experience required for

Qualifications

• Bachelor's degree in Physical Therapy required.
• No experience required.
• Intermediate computer skills.
• Excellent communication and interpersonal skills.
• Demonstrated organization, decision-making, and problem solving skills.

Responsibilities

• Interprets physician referrals and completes musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations utilizing appropriate methods.
• Establishes a plan of care and implements individual treatment based on evaluation results and functional goals.
• Monitors and evaluates outcomes and adjusts the treatment plan based on patient response; provides status updates to physicians.
• Plans for and completes patient discharge including home programs and referrals; orders equipment as needed.
• Documents patient progress notes, reports, discharge summaries, and daily statistics following department policy; applies insurance guidelines for authorization and visits.
• Cleans up after treatment and maintains neat, safe work area; handles lifting and moving with proper technique.
• Demonstrates use of modalities and equipment; knows indications and contraindications of modalities and exercises.
• Participates in performance improvement activities; covers caseload and coordinates with site leadership.
