Job summary

TEEMA Group is seeking a Physical Therapist to provide high-quality, patient-centered care in a hospital-based outpatient rehabilitation department. The PT will evaluate, develop, and implement individualized treatment plans to restore mobility, reduce pain, and improve function, with collaboration across physicians and a multidisciplinary team.

Responsibilities include post-surgical rehab, education on home exercise programs, and accurate EMR documentation.

Qualifications

  • Doctor of Physical Therapy (DPT) or Master’s degree in Physical Therapy from an accredited program.
  • Current, active state Physical Therapist license
  • CPR certification (BLS required)
  • Strong clinical reasoning and evaluation skills
  • Ability to manage a primarily outpatient caseload with some post-surgical patients

Responsibilities

  • Perform comprehensive physical therapy evaluations for outpatient and post-surgical patients
  • Develop individualized treatment plans based on physician referrals and clinical findings
  • Implement evidence-based therapeutic interventions to improve strength, mobility, balance, endurance, and functional independence
  • Provide post-operative rehabilitation for orthopedic and other surgical patients
  • Educate patients and families on home exercise programs, injury prevention, and recovery strategies
  • Document patient evaluations, progress notes, and discharge summaries accurately and timely in the electronic medical record (EMR)
  • Monitor patient progress and adjust treatment plans as necessary to achieve optimal outcomes
  • Collaborate with physicians, surgeons, nursing staff, and the broader healthcare team to coordinate care
  • Ensure compliance with hospital policies, safety standards, and regulatory requirements
  • Maintain a clean, safe, and organized treatment environment
  • Participate in quality improvement initiatives and departmental meetings
  • Perform other related duties as assigned
