Job summary

TEEMA is seeking a Physical Therapist to join a hospital-based outpatient rehabilitation department. The PT will provide patient-centered care, focusing on outpatient and post-surgical rehabilitation, evaluating patients and developing individualized treatment plans to restore mobility, reduce pain, and improve function.

Qualifications

  • DPT or MS in Physical Therapy from an accredited program.
  • Current, active state Physical Therapist license.
  • CPR certification (BLS required).
  • Strong clinical reasoning and evaluation skills.
  • Ability to manage a primarily outpatient caseload with some post-surgical patients.
  • Excellent communication and patient education skills.
  • Proficiency with electronic medical records (EMR) systems.

Responsibilities

  • Perform comprehensive physical therapy evaluations for outpatient and post-surgical patients.
  • Develop individualized treatment plans based on physician referrals and clinical findings.
  • Implement evidence-based therapeutic interventions to improve strength, mobility, balance, endurance, and functional independence.
  • Provide post-operative rehabilitation for orthopedic and other surgical patients.
  • Educate patients and families on home exercise programs, injury prevention, and recovery strategies.
  • Document patient evaluations, progress notes, and discharge summaries accurately and timely in the EMR.
  • Monitor patient progress and adjust treatment plans as necessary to achieve optimal outcomes.
  • Collaborate with physicians, surgeons, nursing staff, and the broader healthcare team to coordinate care.
  • Ensure compliance with hospital policies, safety standards, and regulatory requirements.
  • Maintain a clean, safe, and organized treatment environment.
  • Participate in quality improvement initiatives and departmental meetings.
  • Perform other related duties as assigned.
