Job summary

The physical therapist assesses, plans, organizes and participates in rehabilitative programs that improve mobility, relieve pain, increase strength and decrease or prevent deformity of patients suffering from disease or injury. Services are provided across the full continuum and rendered in a cost conscious, quality focused and customer oriented manner.

Compliance with all applicable regulatory standards is also required in this role.

Qualifications

• Required degrees in physical therapy and licensure to practice.
• Strong communication and teamwork skills.
• Ability to document and manage patient information accurately.

Responsibilities

• Implements the physical therapy treatment plan.
• Selects appropriate treatment activities to progress toward goals.
• Revises goals/plan of care with patient/family input.
• Provides ongoing patient/family education and training.
• Provides discharge instructions and follow-up coordination.
• Acts as a referral source for staff and physicians in rehabilitation services.
• Establishes and evaluates plan of care with patient/family.
• Documents evaluation and plan of care.
• Maintains ethical standards and infection prevention precautions.
