Qualifications

• Licensed Physical Therapist in good standing with current licensure.
• CPR certification current and valid.
• Experience collaborating with a healthcare team.

Responsibilities

• Plan and administer prescribed physical therapy in patients' homes to improve function.
• Assess and document patients' physical strengths to guide goals.
• Develop and implement rehabilitation programs aligned with physician plans.
• Provide clear care instructions to PTAs and Home Health Aides and supervise their work.
• Document patient observations, interventions, and evaluations promptly.
• Report progress to physicians, Clinical Manager, care team, patients and families.
