Job summary

LiveWell Homecare Agency, LLC is seeking a qualified Physical Therapy Assistant to support licensed physical therapists in developing and delivering mobility and rehabilitation programs. You will help restore patients' movement, strength, coordination, and range of motion through supervised treatment sessions.

Responsibilities include implementing therapy plans, educating patients and families, documenting sessions, and maintaining professional standards.

Qualifications

• Associates degree in Physical Therapy or related field.
• Valid PTA license is required.
• Current BLS certification.

Responsibilities

• Implement treatment plans under the physical therapist’s guidance and select appropriate activities to meet patient goals.
• Revise care plans collaboratively and provide patient and family education, discharge instructions, and referrals.
• Document treatment sessions per departmental protocols and complete necessary documentation promptly.
• Maintain ethical standards, confidentiality, punctuality, and proper professional attire.
• Communicate effectively with team members and participate in meetings and training.
• Promote a positive environment, apply customer service principles, and foster teamwork.
• Engage in ongoing professional development, complete required competencies, and participate in orientation and training.
• Adhere to organizational policies, standards, and codes of conduct.
