Job summary

Intermountain Health in Grand Junction, Colorado seeks an Outpatient Clinical Pharmacist to help launch a new ambulatory pharmacy and provide patient-centered care.

You will support the Pharmacy Manager, collaborate with pharmacists and technicians to shape workflows, and ensure safe, efficient medication use in a growing program. This is an opportunity to build outpatient services in a hospital setting.

Qualifications

• Doctor of Pharmacy or BS in Pharmacy from an accredited institution.
• License to practice pharmacy and ability to dispense controlled substances.
• Current knowledge of state and federal regulations governing pharmacy practice.
• Effective communication, people management, and project management skills.
• Immunization certification if applicable to site needs

Responsibilities

• Reviews medication orders for allergies, duplications, interactions and appropriateness; makes interventions to optimize therapy.
• Oversees and ensures timely preparation and delivery of medications to care areas; proper storage.
• Provides counseling to patients and promotes medication education and adherence.
• Supervises interns, technicians, and staff; offers clinical consultation to health professionals.
• Initiates problem solving for equipment, maintenance, drug availability, staffing, and patient care concerns.
• Identifies safety/process improvement opportunities and participates in departmental meetings.
