Job summary

Intermountain Health St. Mary's Regional Hospital in Grand Junction, Colorado, is launching a new outpatient pharmacy and seeks an organized Outpatient Clinical Pharmacist to support daily operations and help shape workflows. You will partner with the Pharmacy Manager, pharmacists, and technicians to deliver safe, patient-centered care.

Qualifications

• Doctor of Pharmacy or BS in Pharmacy from an accredited institution (education verified).
• License to practice pharmacy in the state of practice and dispense controlled substances.
• Current knowledge of state and federal regulations and statutes regarding the practice of pharmacy.
• Effective communication, people management, and project management skills.
• Immunization certification if applicable to practice site needs

Responsibilities

• Reviews patients' medication orders for allergy interactions, duplication of therapy, appropriateness of drug therapy, and drug-drug interactions. Makes evidence-based interventions to optimize medication use and dispense safely.
• Oversees and/or ensures medications are prepared and delivered timely to care areas, stored securely and appropriate for stability and workflow.
• Counsels patients and promotes medication education and adherence; ensures patients are well informed and receive high-quality service.
• Supervises interns, technicians, and support personnel; provides clinical consultation to health professionals.
• Identifies issues and initiates problem-solving for equipment, maintenance, drug availability, staffing, and patient care concerns.
• Suggests process improvements to enhance patient satisfaction and safety; participates in departmental meetings with constructive input
