Job summary

Fresenius Medical Care in Lincolnwood, IL seeks an Onsite Outpatient Registered Nurse (RN) for an entry-level CAP position. The role focuses on assessment, planning, intervention, and evaluation for a patient group, with supervision and team collaboration in dialysis care.

Responsibilities include data collection around dialysis, patient education, and delivering safe hemodialysis therapy. Training is provided, with potential for CAP advancement.

Qualifications

• Graduate of an accredited School of Nursing.
• Current state licensure.
• Current or successful CPR BLS Certification.
• Must complete CAP requirements to advance.

Responsibilities

• Collect and analyze patient data during pre-, during-, and post-dialysis and adjust the care plan as needed.
• Educate patients and families on learning needs and treatment options.
• Deliver safe hemodialysis therapy with the care team.
• Administer medications as prescribed and document rationale and effectiveness.
• Initiate or assist with emergency response measures as required.
• Supervise and orient new staff as assigned.
• Ensure proper lab collection, processing and shipping; reschedule missed labs.
• Identify expected outcomes and update nursing assessments and plan of care with the Interdisciplinary Team.
• Explain transplantation and treatment modality options to patients.

Education

Graduate of an accredited School of Nursing

Current appropriate state licensure
