• Pre-, during-, post-hemodialysis data collection and documentation in the medical record; adjust care plan as needed.
• Educates patients and families about care plans and treatment options.
• Directs daily safe delivery of hemodialysis therapy in collaboration with care staff.
• Administers prescribed medications and documents effectiveness.
• Participates in emergencies and quality improvement activities.
• Coordinates with Interdisciplinary Team to update plans of care.
