Job summary

Fresenius Medical Care is seeking an entry level Registered Nurse for the Outpatient CAP program. The role focuses on coordination of nephrology care, patient assessment, planning, and evaluation within the End Stage Kidney Disease team.

Responsibilities include data collection, education for patients and families, medication administration, and collaboration with the interdisciplinary team to ensure safe, quality dialysis therapy.

Qualifications

• Entry level RN with less than 2 years of nephrology nursing experience.
• Chronic/acute hemodialysis experience preferred.

Responsibilities

• Provide and coordinate clinically competent care including assessment, planning, intervention and evaluation for an assigned group of patients.
• Document patient data and updates to care plans in the medical record.
• Administer medications as prescribed and participate in emergency responses.
• Lead or supervise Patient Care Technicians/LPNs/RNs as assigned.
• Support education for patients and families and collaborate with the health care team.
