Job summary

Fresenius Medical Care in Kentucky is seeking an entry level RN to join the Outpatient CAP 1 program. You will assess, plan, intervene and evaluate care for an assigned patient group, coordinate with RNs, LVN/LPNs and PCTs, and support ESRD team activities.

Responsibilities include data collection during dialysis, patient education, medication administration, and collaboration with the interdisciplinary team to update care plans.

Qualifications

• Graduate of an accredited School of Nursing.
• Current appropriate state licensure.
• Current CPR BLS Certification.
• Must meet the practice requirements in the state employed.

Responsibilities

• Performs duties under supervision with guidance from Clinical Educator/Preceptor or RN.
• Collects and analyzes patient data during dialysis and documents in medical records; adjusts care plan as needed.
• Educates patient and family on care needs.
• Delivers safe and effective hemodialysis therapy in collaboration with the team.
