Job summary

Fresenius Medical Care seeks an entry-level Registered Nurse for the Outpatient CAP program. The role focuses on coordinating and providing clinically competent care, including assessment, planning, intervention and evaluation for an assigned patient group within the End Stage Kidney Disease team.

You will collaborate with RNs, LVNs/LPNs and Patient Care Technicians, participate in teaching, leadership activities, and quality improvements to enhance patient outcomes and facility operations.

Qualifications

• Graduate of an accredited School of Nursing.
• Current appropriate state licensure.
• Current or successful completion of CPR BLS Certification.
• Must meet the practice requirements in the state in which he or she is employed.

Responsibilities

• Perform assessment, planning, intervention and evaluation for an assigned group of patients.
• Delegate tasks to direct patient care staff as appropriate.
• Coordinate hemodialysis therapy and ensure safe delivery of care.
• Administer medications as prescribed.
• Provide education to patients and families.
• Participate in quality improvement activities.
