Qualifications

Responsibilities

• Provides and coordinates clinically competent care including assessment, planning, intervention and evaluation for assigned patients.
• Administers medications as prescribed or per approved algorithm and documents justification and effectiveness.
• Directs delivery of safe hemodialysis therapy and collaborates with care staff.
• Educates patients and families about care plans and transplant/treatment options.
• Collaborates with interdisciplinary team and participates in staff training and quality improvement.
