Job summary

Physician Health Partners is seeking an RN Care Coordinator to deliver comprehensive care coordination within Primary Care settings in the Denver area. You will assess, plan and monitor care for at-risk members, collaborate with PC providers, and facilitate transitions across the care continuum.

The role emphasizes member-centered care, effective communication, and participation in care rounds, with opportunities to improve patient outcomes and satisfaction.

Qualifications

• RN with valid license in good standing and current licensure requirements.
• Experience with high-risk DSNP/CSNP or Medicaid populations preferred.
• Minimum of one year in case management or related healthcare roles.
• Experience supporting transitions across the care continuum.
• Knowledge of case management concepts and community resource navigation.

Responsibilities

• Communicate assessment findings, care plan goals, interventions and outcomes to PC providers in a timely manner.
• Develop and implement care plans in coordination with members, caregivers and care teams.
• Participate in Care Rounds and provide member reports and updates to the care team.
• Collaborate with providers to identify high-risk members and plan care path.
• Document and update goals, interventions and outcomes in EMR per SOPs.
