Job summary

Banner Health Corporate in Fort Collins, Colorado is seeking an RN Case Manager to join the CO Case Mgmt-BMG team. You will manage a caseload of high-risk patients, perform assessments, develop care plans, review medications, and coordinate with an interdisciplinary care team.

This hybrid role requires travel in Northern Colorado and a Monday–Friday schedule. Candidates must have 2+ years in case management, an R.N. license, BLS, and a bachelor’s degree in case management or health care.

Qualifications

• Bachelor’s degree in case management or health care (minimum).
• Current RN license in the state worked; BLS certification required for acute care settings.
• 3–5 years of clinical experience in case management, acute care, or home care.
• Strong communication, critical thinking, problem solving, and time management; ability to work in an interdisciplinary team.

Responsibilities

• Manage a caseload of high‑risk patients and coordinate care.
• Conduct assessments and develop individualized care plans.
• Review medications and connect patients with resources.
• Collaborate with an interdisciplinary care team and external providers.
• Educate patients on chronic disease management.
• Travel to Northern Colorado as needed; hybrid work model.
