Qualifications

• Bachelor's degree in case management or health care.
• RN license in state worked is required.
• BLS certification required for acute care assignments.
• 3–5 years of clinical experience preferred.
• Working knowledge of care management, acute care and/or home care environments.

Responsibilities

• Manages individual patients across the health care continuum to achieve optimal outcomes.
• Leads process improvement activities for populations of patients.
• Develops and manages interdisciplinary patient discharge plan and communicates it across care continuum.
• Evaluates medical necessity and appr opriateness of care to optimize outcomes.
• Collaborates with physicians, payers and health care team to meet utilization and regulatory requirements.
• Educates health care team on case management concepts and managed care.
• May supervise other staff.
• Works within established procedures with supervisor guidance.
