Qualifications

• Knowledge of case management or utilization review.
• RN license in state worked; BLS certification as required.
• 3–5 years clinical experience in care management, acute or home care.
• Strong critical thinking, communication, and time management.
• Ability to work in an interdisciplinary team; flexible hours.

Responsibilities

• Manage individual patients across health care continuum to achieve optimal clinical, financial, operational, and satisfaction outcomes.
• Lead process improvement activities for populations of patients to improve outcomes.
• Collaborate to develop and manage interdisciplinary patient discharge plans and communicate across the care continuum.
• Evaluate medical necessity and optimize patient outcomes; assess admissions and discharge barriers.
• Establish relationships with physicians, payers, and health care team; communicate data for utilization and regulatory requirements.
• Educate health care team on case management concepts and integrate into daily practice.
• May supervise other staff; work within established procedures; no budget responsibility.
