Job summary

CentralReach, LLC is seeking a Outcomes-Based Care Manager to guide provider organizations in shifting to outcomes-based care models using our platform and AI-enabled tools. You will collaborate with clinical and executive leaders to implement frameworks, interpret data, and support payer relationships.

This hybrid, customer-facing role reports to the VP, Payor Strategy and Network Development, and requires BCBA certification and 4+ years of clinical ABA/IDD experience.

Qualifications

• Active BCBA certification or equivalent clinical credential in autism/IDD care.
• 4+ years of clinical experience in ABA or IDD services, including supervisory or leadership roles.
• Experience advising, training, or consulting external clients or stakeholders.
• Strong understanding of clinical data collection, progress monitoring, and how treatment data translates to outcomes.
• Ability to interpret data and communicate findings clearly to clinical and executive audiences.
• Comfort with software platforms and data tools; curiosity about applying AI to clinical/operational work.
• Ability to manage multiple customer engagements independently.

Responsibilities

• Serve as the clinical outcomes expert for assigned customers, advising on adopting outcomes-based care models.
• Lead outcomes-focused onboarding, implementation, and optimization engagements, including sessions and reviews.
• Assess customers’ data practices and outcomes maturity; build a roadmap to close gaps.
• Deliver training and best-practice guidance for applying outcomes measurement in workflows.
• Configure reporting and dashboards reflecting clinically sound outcomes methodology.
• Help customers build outcomes reports and narratives for payers, funders, and families.
• Advise on value-based and outcomes-linked arrangements with quality metrics.
