Job summary

CentralReach is seeking an Outcomes-Based Care Manager to work with provider organizations on shifting to outcomes-based care using the company’s platform. You will interpret progress data, implement outcomes frameworks, and engage clinical and executive leaders to demonstrate value-based payer relationships.

You will collaborate with Payor Strategy and Product teams to scale evidence-based approaches, while maintaining clinical judgment and ethical oversight.

Qualifications

• Active BCBA certification (or equivalent clinical credential) in autism/IDD care.
• 4+ years of clinical experience in ABA or IDD services, including supervisory or leadership roles.
• Experience advising, training, or consulting external clients or stakeholders.
• Strong understanding of clinical data collection and progress monitoring.
• Ability to interpret data and clearly communicate findings to clinical and executive audiences.
• Comfort with software platforms and data tools; openness to AI-enabled clinical workflows.
• Ability to manage multiple customer engagements independently.

Responsibilities

• Serve as the clinical outcomes expert for assigned customers and guide adoption of outcomes-based care models.
• Lead outcome-focused onboarding, implementation, and optimization engagements with discovery sessions and reviews.
• Assess customers' data practices and build practical roadmaps to close gaps.
• Provide training and best-practice guidance for applying outcomes measurement in daily workflows.
