Job summary

CentralReach, a leading provider of autism and IDD care software, seeks an Outcomes-Based Care Manager. You will partner with clinical and executive leaders to implement outcomes frameworks, interpret progress data, and build evidence for value-based payer relationships using CentralReach’s platform and AI-enabled tools.

This is an individual contributor role reporting to the VP, Payor Strategy and Network Development, leading onboarding, measurement, and collaboration with Customer Success,

Qualifications

• Active BCBA certification in good standing, or an equivalent clinical credential.
• 4+ years of clinical experience in ABA or IDD services, including supervisory or quality-focused work.
• Experience advising, training, or consulting with external clients or stakeholders.
• Strong understanding of clinical data collection, progress monitoring, and how treatment data translates to outcomes.
• Ability to interpret data and communicate findings clearly to clinical and executive audiences.
• Comfort with software platforms and data tools, and curiosity about applying AI to clinical and operational work.
• Ability to manage multiple customer engagements independently.

Responsibilities

• Serve as the clinical outcomes expert for assigned customers and advise on adopting outcomes-based care models.
• Lead onboarding, implementation, and optimization engagements with discovery sessions and reviews.
• Assess customers' data practices and build practical roadmaps to close gaps.
• Deliver training and best-practice guidance for applying outcomes measurement in workflows.
