Creative Solutions in Healthcare, Inc. is seeking a dedicated Occupational Therapist to join our in-house rehab team in Texas.

This role focuses on improving residents’ independence and safety through ADLs/IADLs, with person-centered care and collaborative treatment planning with the interdisciplinary team. Full-time or PRN opportunities in a SNF/long-term care setting, requiring active Texas OT license and experience with adaptive equipment and documentation.

Qualifications

• Currently licensed as an OT in Texas and in good standing.
• SNF/long-term care experience preferred.
• Knowledge of OT treatment practices and safety standards.

Responsibilities

• Consult with patients to evaluate physical condition and daily occupations.
• Assess and interpret patient evaluations and test results.
• Develop individualized treatment plans to maximize ADLs and IADLs.
• Assess home environments and recommend modifications based on needs.
• Recommend adaptive equipment and provide training for independence and safety.
• Deliver treatments per the established plan of care.
• Monitor patient response and modify interventions with the interdisciplinary team.
• Provide education to patients, families, and caregivers.
• Collaborate with interdisciplinary team to ensure high-quality care.
• Complete documentation in compliance with policies and regulations.
• Attend required meetings as designated by the Director of Rehab.
