Job summary

Creative Solutions in Healthcare - East is seeking a compassionate Occupational Therapist (OT) for a PRN role in Texas. You will evaluate patients, create and implement individualized treatment plans, and work with an interdisciplinary team to maximize independence in ADLs/IADLs.

Texas license in good standing is required; SNF or long-term care experience is preferred. Familiarity with Net Health is a plus; flexible schedules are available.

Qualifications

• Active and valid Texas Occupational Therapy license
• In good standing with regulatory agencies and licensing boards
• SNF or long-term care experience preferred
• Current knowledge of occupational therapy treatment practices
• Experience with Net Health Electronic Documentation System preferred
• Ability to manage patients with varying personalities and needs
• Strong communication, collaboration, and clinical decision-making skills

Responsibilities

• Consult with patients to evaluate their physical condition and its impact on daily occupations, well-being, and quality of life
• Assess and interpret patient evaluations and test results
• Develop individualized treatment plans using a variety of therapeutic techniques to maximize performance in ADLs and IADLs
• Assess patients’ home and living environments and recommend modifications
• Recommend adaptive equipment and provide training to support independence and safety
• Ensure all treatments are delivered in accordance with the plan of care
• Monitor patient response to treatment and modify interventions in collaboration with the interdisciplinary team
• Provide clinical education and support to patients, family members, and caregivers
• Collaborate with interdisciplinary team members to ensure high-quality patient care
• Complete documentation thoroughly and timely in compliance with policies and regulations
• Comply with all applicable rules, regulations, and procedures
• Attend required meetings as designated by the Director of Rehab
