Job summary

La Hacienda De Paz Rehabilitation and Care Center in Texas seeks a dedicated Occupational Therapist to join our in-house team. This full-time role focuses on helping residents maintain independence and safety through meaningful daily activities, in collaboration with an interdisciplinary team.

You will evaluate patients, develop individualized treatment plans, and document progress in Net Health. SNF/long-term care experience and a current Texas OT license are preferred to ensure high-quality

Qualifications

• Active and valid Texas Occupational Therapy license.
• In good standing with regulatory agencies and licensing boards.
• SNF or long-term care experience preferred.
• Current knowledge of occupational therapy treatment practices.
• Experience with Net Health Electronic Documentation System preferred.
• Ability to manage patients with varying needs and personalities.
• Strong communication, collaboration, and clinical decision-making skills.

Responsibilities

• Evaluate patients to determine impact on daily occupations and quality of life.
• Assess and interpret patient evaluations and test results.
• Develop individualized treatment plans to maximize ADL/IADL performance.
• Assess home environments and recommend safety modifications.
• Recommend adaptive equipment and provide training for independence.
• Deliver treatments per care plan and monitor responses.
• Document progress thoroughly and timely in accordance with policies.
• Collaborate with interdisciplinary team to ensure high-quality care.
