Creative Solutions in Healthcare, Inc. in Texas is hiring a dedicated Occupational Therapist (OT) to join our in-house rehab team. You will evaluate residents, develop individualized treatment plans, and support daily living activities to improve independence and safety.

Qualifications

• Active and valid Texas Occupational Therapy license.
• SNF or long-term care experience preferred.
• Knowledge of OT treatment practices and evidence-based approaches.
• Experience with electronic medical records systems preferred.

Responsibilities

• Evaluate patients' condition and impact on daily occupations to improve independence.
• Develop personalized treatment plans to maximize ADLs and IADLs.
• Assess home environments and recommend safety modifications.
• Provide patient and caregiver education and training.
• Record timely documentation per policy and state/federal regulations.
• Collaborate with the interdisciplinary team to ensure high-quality care.
