Job summary

Autumn Lake at Salem County in Salem, NJ is seeking a PRN Occupational Therapist to evaluate residents, develop individualized goals, and provide task-based therapy for daily activities. You will supervise COTAs, ensure accurate documentation, and coordinate with the care team to optimize functional outcomes.

Applicants must hold an active NJ OT license. New graduates are welcome, with opportunities for mentorship and collaboration with nursing and rehabilitation staff in a skilled nursing

Qualifications

• Active New Jersey Occupational Therapist license.
• Experience in skilled nursing or post-acute settings preferred; new graduates welcome.

Responsibilities

• Evaluate daily living performance, upper-extremity function, cognition, and endurance to determine treatment needs.
• Create individualized goals considering residents' prior abilities, current challenges, and discharge plans.
• Provide task-based therapy for dressing, bathing, toileting, and functional transfers.
• Assess adaptive equipment needs and train residents and caregivers in safe techniques for task completion.
• Supervise COTA treatment, review outcomes, and modify plans as residents progress.
• Maintain accurate clinical records and discuss care recommendations with nursing and other professionals.

Stern Therapy Consultants is looking for a PRN Occupational Therapist at Autumn Lake at Salem County. Help residents receiving skilled nursing rehabilitation find effective ways to manage personal care, rebuild functional abilities, and participate in the routines that matter to them.

What You'll Do

• Evaluate daily living performance, upper-extremity function, cognition, and endurance to determine treatment needs.
• Create individualized goals that account for residents' prior abilities, current challenges, and discharge plans.
• Provide task-based therapy for dressing, bathing, toileting, and functional transfers.
