Job summary

Sternrehab is seeking a PRN Occupational Therapist to join Autumn Lake at Memorial Bridge in Carneys Point, NJ. You will evaluate residents, develop and implement treatment plans, and educate families and staff. This role offers PRN coverage with competitive hourly pay and strong team support.

Responsibilities include direct COTA supervision, progress reviews, and adapting plans to discharge environments, with emphasis on safety and independence in daily tasks.

Qualifications

• Active New Jersey Occupational Therapist license.
• Strong clinical assessment and treatment planning skills.
• Clear communication and accurate, timely documentation.
• Experience in skilled nursing or post-acute rehabilitation is a plus; new grads welcome.

Stern Therapy Consultants is seeking a PRN Occupational Therapist at Autumn Lake at Memorial Bridge. Help skilled nursing residents approach daily tasks with greater confidence through individualized rehabilitation that combines skill development, adaptive strategies, and caregiver education.

What You’ll Do

• Evaluate self‑care abilities, upper‑extremity function, cognition, and endurance to identify barriers to daily activity.
• Build treatment plans around residents’ functional needs, personal goals, and expected discharge environment.
• Provide interventions for dressing, bathing, toileting, and transfers associated with daily routines.
• Assess adaptive equipment needs and teach practical techniques for safer task performance.
• Direct COTA services, review resident progress, and revise plans of care when appropriate.
• Document evaluations and treatment, communicating recommendations to families, nursing, and rehabilitation colleagues.
