Job summary

Continuum Therapy Partners, LLC is seeking an Occupational Therapist to join our Wisconsin Rapids sites. You will provide patient-centered OT services at Wisconsin Rapids Health Services and River Run Assisted Living, collaborating with a multidisciplinary team to help residents improve functional independence and quality of life.

Required are a Master’s or Doctorate in Occupational Therapy, an active state OT license, NBCOT certification, and preferred 1 year of OT experience.

Qualifications

• Master's or Doctorate Degree in Occupational Therapy.
• Current, unencumbered state OT license; NBCOT Certification.
• 1 year of previous experience as an Occupational Therapist preferred.

Responsibilities

• Evaluates and develops individualized treatment plans to improve patients’ functional independence.
• Documents patient progress and modifies treatment plans based on clinical outcomes.
• Educates patients and families on adaptive equipment and home safety recommendations.
