Continuum Therapy Partners in Wisconsin is seeking an Occupational Therapist to join our team at Wausau Manor Health Services. You will assess patients, develop individualized treatment plans, and collaborate with a multidisciplinary team to improve functional independence.

Qualifications

• Master's or Doctorate Degree in Occupational Therapy required.
• Current, unencumbered state Occupational Therapy License; NBCOT Certification required.
• 1 year of previous experience as an Occupational Therapist preferred.
• Evaluates and develops individualized treatment plans to improve patients' functional independence.
• Documents patient progress and modifies treatment plans based on clinical outcomes.
• Educates patients and families on adaptive equipment and home safety recommendations.
