Job summary

Continuum Therapy Partners, LLC is seeking an Occupational Therapist in Lancaster, Wisconsin. This full-time daytime role focuses on delivering patient-centered care, evaluating and addressing functional independence in collaboration with a multidisciplinary team.

The ideal candidate holds a Master's or Doctorate in OT, a current state license and NBCOT certification, and prefers at least one year of experience.

Qualifications

• Master's or Doctorate Degree in Occupational Therapy required.
• Current, unencumbered state Occupational Therapy License; NBCOT Certification required.
• 1 year of previous experience as an Occupational Therapist preferred.

Responsibilities

• Evaluates and develops individualized treatment plans to improve patients’ functional independence.
• Documents patient progress and modifies treatment plans based on clinical outcomes.
• Educates patients and families on adaptive equipment and home safety recommendations.
