Job summary

Flagstar Rehab is seeking an Occupational Therapy Assistant (OTA) for Skilled Nursing Facilities in New York. You will collaborate with OT supervisors to deliver patient-centered therapy and document progress in an SNF setting.

We offer flexible scheduling options (FT, PT, per diem) with weekly direct deposit, paid sick leave, and optional insurance; experienced candidates with NYS OTA license and BLS are preferred. Hourly pay ranges from $34-$36 (W-2) or $38-$40 (1099), with weekly pay.

Qualifications

• Current NYS OTA license and registration required.
• Valid BLS Certification.
• Proficiency with EMR systems (REHAB OPTIMA and/or POINT CLICK CARE a plus).
• Meets all State and Federal requirements for US employment in a Nursing Facility.
• Malpractice/Liability Insurance (if applicable).

Responsibilities

• Collaborate daily with OT Supervisor and follow established Plan of Care for all patients.
• Provide quality therapy services to individuals in an SNF setting with various diagnoses.
• Monitor and document patient progress.

Hourly Pay Rate: $34-$36 Hourly (W-2), $38-$40 Hourly (1099)

Occupational Therapy Assistant Requirements and Responsibilities:

• Current NYS OTA License and Registration
• Valid BLS Certification
• EMR Competence (Proficiency in REHAB OPTIMA + POINT CLICK CARE a plus!)
• Meets all State and Federal requirements for US employment in a Nursing Facility

• Provide quality therapy services to individuals in an SNF setting who require care related to TBI, CVA, Stroke, Parkinson's, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress
