Job summary

Flagstar Rehab is seeking Occupational Therapists (OT) for Skilled Nursing Facilities. The role offers flexible scheduling and remote onboarding to fit your lifestyle while delivering quality patient care in SNF settings.

Earn $46–$48 per hour as a W-2 or $57–$60 per hour as a 1099 contractor, with full-time, part-time, per diem, and weekend options, plus benefits like paid orientation, sick leave, insurance options, commuter benefits, direct deposit, referral bonuses, and 24/7 support.

Qualifications

• Must hold NYS OT license and registration.
• BLS certification required.
• Experience using EMR systems.

Responsibilities

• Provide efficient evaluations and create individualized treatment plans for COTAs to follow under supervision.
• Deliver quality therapy services in SNF settings for conditions such as TBI, CVA, Stroke, Parkinson’s, MS, cancer and orthopedic problems.
• Monitor and document patient progress using EMR software.
• Collaborate with supervising therapists to ensure high-quality patient care.

Pay Rate: $46-$48 Hourly (W-2), $57-$60 Hourly (1099)
Occupational Therapist Requirements and Responsibilities:

• Current NYS OT License and Registration
• Valid BLS Certification
• EMR Competence
• Meets all State and Federal requirements for US employment in a Nursing Facility
• Malpractice/Liability Insurance (if applicable)
• Provide efficient evaluations and create individualized treatment plans for COTAs to follow under your supervision.
• Provide quality therapy services to individuals in an SNF setting who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress with the use of innovative EMR software.
