Job summary

Flagstar Rehab is seeking Occupational Therapy Assistants (OTAs) for Skilled Nursing Facilities in New York. We offer flexible earning models and quick onboarding, with remote onboarding supported.

Position includes Full-Time, Part-Time, Per Diem shifts; benefits include paid orientation, sick leave, and optional insurance, with weekly pay and referral bonuses. Applicants must hold NYS OTA license and BLS certification.

Qualifications

• Must hold NYS OTA license and registration.
• BLS certification required.
• Experience with EMR systems is preferred; REHAB OPTIMA and POINT CLICK CARE proficiency a plus.

Responsibilities

• Collaborate daily with OT Supervisor and follow established Plan of Care for all patients.
• Provide quality therapy services to SNF residents with geriatric conditions such as TBI, CVA, Stroke, Parkinson’s, MS, cancer, orthopedic surgery.
• Monitor and document patient progress.

Hourly Pay Rate

$34-$36 Hourly (W-2), $38-$40 Hourly (1099)

Occupational Therapy Assistant Requirements and Responsibilities

• Current NYS OTA License and Registration
• Valid BLS Certification
• EMR Competence (Proficiency in REHAB OPTIMA + POINT CLICK CARE a plus!)
• Meets all State and Federal requirements for US employment in a Nursing Facility
• Malpractice/Liability Insurance (if applicable)
• Collaborate daily with OT Supervisor and follow established Plan of Care for all patients.
• Provide quality therapy services to individuals in an SNF setting who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress
