Job summary

Continuum Therapy Partners, LLC is seeking an Occupational Therapist to join our therapist-led team. We value clinical expertise and patient-centered care, helping patients achieve functional independence in daily life.

The role requires a Master’s or Doctorate in Occupational Therapy, an active state license, NBCOT certification, and a preference for 1 year of OT experience. PRN daytime shifts apply, with on-site locations including Streetsboro, Ohio.

Qualifications

• Master’s or Doctorate Degree in Occupational Therapy required.
• Current, unencumbered state OT license; NBCOT certification required.
• 1 year of previous experience as an Occupational Therapist preferred.

Responsibilities

• Evaluates and develops individualized treatment plans to improve patients’ functional independence.
• Documents patient progress and modifies treatment plans based on clinical outcomes.
• Educates patients and families on adaptive equipment and home safety recommendations.
