Job summary

Continuum Therapy Partners, LLC is seeking an Occupational Therapist to join a therapist-led team expanding across the country. The role emphasizes evidence-based therapy planning and direct patient care at Montgomery Nursing & Rehab Center in Hillsboro, IL.

The position offers PRN daytime scheduling and emphasizes collaboration with a leadership team that values your professional perspective and expertise in improving functional independence for patients.

Qualifications

• Master's or Doctorate Degree in Occupational Therapy required.
• Current, unencumbered state Occupational Therapy License; NBCOT Certification required.
• 1 year of previous experience as an Occupational Therapist preferred.

Responsibilities

• Evaluates and develops individualized treatment plans to improve patients’ functional independence.
• Documents patient progress and modifies treatment plans based on clinical outcomes.
• Educates patients and families on adaptive equipment and home safety recommendations.

Education

Occupational Therapist Requirements & Responsibilities
