Job summary

Continuum Therapy Partners is seeking an Occupational Therapist to evaluate patients, develop individualized treatment plans, and help patients gain functional independence within our care team at Fifth Avenue Health Care Center - Rome, GA. Qualified candidates hold a Master's or Doctorate in Occupational Therapy, an active GA license, NBCOT certification, and may have 1 year of experience.

• Documents patient progress and modifies treatment plans based on clinical outcomes.
• Educates patients and families on adaptive equipment and home safety recommendations.
