Job summary

Flagstar Rehab is actively seeking Occupational Therapists to work in skilled nursing facilities, offering flexible full-time, part-time and per diem schedules. We provide W-2 pay at $46-$48 hourly or 1099 options at $57-$60 hourly, with remote onboarding and quick placement to minimize downtime between assignments.

Requirements include an active NYS OT license and registration, valid BLS, and EMR competence.

Responsibilities

• Provide efficient evaluations and create individualized treatment plans under supervision

Pay Rate: $46-$48 Hourly (W-2), $57-$60 Hourly (1090)

Occupational Therapist Requirements and Responsibilities:

• Current NYS OT License and Registration
• Valid BLS Certification
• EMR Competence
• Meets all State and Federal requirements for US employment in a Nursing Facility
• Malpractice/Liability Insurance (if applicable)
• Provide efficient evaluations and create individualized treatment plans for COTAs to follow under your supervision.
• Provide quality therapy services to individuals in an SNF setting who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress with the use of innovative EMR software.
