Flagstar Rehab is seeking Occupational Therapists for Skilled Nursing Facilities to provide high-quality therapy and assessments. We offer flexible assignments with options for W-2 or 1099 arrangements and ongoing support.

Requirements include NY license, BLS, and EMR proficiency, with opportunities to work in SNFs and treat diverse conditions such as stroke, TBI, and orthopedic cases. Weekly pay and benefits enhance work-life balance.

Qualifications

• Current NYS OT License and Registration.
• Valid BLS Certification.
• EMR Competence.
• Meets all State and Federal requirements for US employment in a Nursing Facility.
• Malpractice/Liability Insurance (if applicable).

Responsibilities

• Provide efficient evaluations and create individualized treatment plans for COTAs to follow under your supervision.
• Provide quality therapy services to individuals in a SNF setting who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress with the use of innovative EMR software.

Pay Rate: $46-$48 Hourly (W-2), $57-$60 Hourly (1099)
