Job summary

Flagstar Rehab is seeking Occupational Therapists (OT) for Skilled Nursing Facilities (SNFs) in New York. The role offers flexibility to choose assignments that fit your schedule, with a “choose the way you earn” model and independent work options.

Pay rates are $46-$48 per hour for W-2 and $57-$60 per hour for 1099, with full-time, part-time, per diem, weekends, days, evenings, and shifts available. Weekly pay and 24/7 recruiter support are provided.

Qualifications

• Current NYS OT License and Registration
• Valid BLS Certification
• EMR Competence
• Meets all State and Federal requirements for US employment in a Nursing Facility
• Malpractice/Liability Insurance (if applicable)

Responsibilities

• Provide efficient evaluations and create individualized treatment plans for COTAs to follow under your supervision.
• Provide quality therapy services to individuals in a SNF setting who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress with the use of EMR software.
