Job summary

Flagstar Rehab is seeking licensed Occupational Therapists to join skilled nursing facilities nationwide. You will assess patients, develop individualized treatment plans, and supervise COTAs to ensure high-quality care.

We offer flexible schedules, weekly pay, and a range of benefits. Independent contractors are handled with specific terms; W-2 or 1099 options available, with ongoing support and onboarding streamlined for clinicians.

Qualifications

• Active NYS OT license and registration.
• Valid BLS certification is required.
• Experience with EMR systems or software.
• Authorized to work in the US in a Nursing Facility.

Responsibilities

• Provide efficient evaluations and create individualized treatment plans for COTAs to follow under supervision.
• Deliver quality therapy services to SNF patients with a variety of conditions.
• Monitor and document patient progress using EMR software.

Pay Rate: $46-$48 Hourly (W-2), $57-$60 Hourly (1099)

Occupational Therapist Requirements and Responsibilities:

• Current NYS OT License and Registration

• Valid BLS Certification

• EMR Competence

• Meets all State and Federal requirements for US employment in a Nursing Facility

• Malpractice/Liability Insurance (if applicable)

• Provide efficient evaluations and create individualized treatment plans for COTAs to follow under your supervision.

• Provide quality therapy services to individuals in an SNF setting who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.

• Monitor and document patient progress with the use of innovative EMR software.
