Job summary

Evolve Therapy Services is seeking an Occupational Therapist for an in-house Skilled Nursing Facility in Olivia, MN. The role includes planning, evaluating, and delivering occupational therapy to residents, with an emphasis on collaborative care and compliant documentation.

The position offers a full-time option (also part-time or PRN) with a $5,000 sign-on bonus and tuition reimbursement, plus competitive benefits and opportunities for career growth.

Qualifications

• Degree in Occupational Therapy.
• Active Occupational Therapy license in practicing state(s).
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Conduct regular screening of residents to determine best interventions.
• Evaluate residents to develop treatment plans and goals.
• Communicate evaluation findings and treatment plans to residents, families and team.
• Establish individual resident therapy schedules with the team.
• Deliver therapy per treatment plan and ensure proper documentation and billing.
• Perform other duties as required to treat patients.
• Oversee COTA as needed and ensure documentation and reimbursement practices.
