Job summary

Evolve Therapy Services is hiring an Occupational Therapist for an in-house Skilled Nursing Facility partner located in Olivia, MN. You will plan, organize, develop, and direct OT services to ensure high-quality resident care and regulatory compliance.

The role offers full-time, part-time, or PRN options with a $5,000 sign-on bonus and tuition reimbursement for approved education. Active licensure and ongoing professional development are required to maintain licensure.

Qualifications

• Degree in Occupational Therapy is required.
• Active OT license in practicing state(s) is required.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Conduct regular screening of residents to determine best interventions and treatments for each patient.
• Evaluate residents to inform and develop treatment plans with short- and long-term goals.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families, and the interdisciplinary team.
• Establish individual resident therapy schedules in coordination with other team members.
• Perform therapy according to treatment plans and ensure documentation and billing of OT services.
• Oversee COTA as needed and ensure documentation and treatments meet reimbursement and clinical standards.

Education

Degree in Occupational Therapy

Active OT license in practicing state(s)

Ongoing continuing education and professional development
