Job summary

Evolve Therapy Services is seeking an Occupational Therapist for an in-house Skilled Nursing Facility in Olivia, MN. This full-time role includes a $5,000 sign-on and tuition reimbursement along with part-time and PRN options.

Responsibilities include screening, evaluating, planning, implementing, and documenting therapy services; coordinating with the interdisciplinary team; and supervising COTAs to ensure quality resident care and proper reimbursement practices.

Qualifications

• Degree in Occupational Therapy.
• Active Occupational Therapy license in practicing state(s).
• Ongoing continuing education to maintain current licensure and certification.

Responsibilities

• Conduct regular screening of residents to determine best intervention or treatment.
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families, and team members.
• Establish individual resident therapy schedules in cooperation with other team members.
• Conduct therapy according to treatment plan and ensure proper documentation and billing.
• Perform all other duties as assigned to effectively treat patients.
• Oversee COTA as needed and ensure documentation and treatments meet reimbursement practices.

Education

Degree in Occupational Therapy

Active Occupational Therapy license in practicing state(s)

Continuing education to maintain licensure
