Job summary

Evolve Therapy Services is hiring compassionate occupational therapists for an in-house Skilled Nursing Facility in Olivia, MN. The OT will plan, organize, develop, and direct therapy services to ensure the highest quality resident care in line with standards.

Qualifications

• Degree in Occupational Therapy.
• Active OT license in practicing state(s).
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to determine best intervention or treatment.
• Evaluate residents to inform treatment plans and goals.
• Communicate findings and recommendations to residents, families, and team.
• Set therapy schedules with team members.
• Document and bill OT services per plan.
• Perform duties to effectively treat patients.
• Oversee COTA and ensure documentation and reimbursement practices are followed.
